We are dedicated to fostering a supportive environment that values continuous learning and professional growth. *Duties* * Conduct comprehensive assessments of patients? physical conditions, utilizing your deep knowledge of physiology and anatomy to inform treatment strategies. * Develop and implement tailored care plans that address individual needs, including specialized therapies such as trigger point therapy and orthopedics. * Deliver effective physiotherapy interventions across various settings?hospital inpatient units, outpatient clinics, home health visits, and community programs?ensuring seamless patient care transitions. * Perform discharge planning by coordinating with multidisciplinary teams to facilitate safe and efficient patient transitions from hospital to home or community care. * Engage in patient education to promote self-management techniques, injury prevention, and rehabilitation exercises. * Maintain accurate documentation within electronic medical record (EMR) systems, ensuring compliance with healthcare standards and regulatory requirements. * Collaborate with healthcare professionals to optimize treatment outcomes for patients across geriatrics, pediatrics, orthopedics, and other specialties. *Requirements* * Valid registration as a Physiotherapist with the appropriate licensing body in your region. * Proven experience in acute care settings such as hospitals or inpatient units, with familiarity in handling complex medical cases. * Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques relevant to diverse patient populations. * Demonstrated ability to develop comprehensive care plans incorporating discharge planning and community resources. * Experience working with EMR systems for accurate documentation and efficient workflow management. * Background in home & community care environments or home health is highly desirable. * Excellent communication skills to effectively educate patients and collaborate with multidisciplinary teams. * Ability to adapt treatment approaches for outpatient clinics or outpatient therapy settings.
