*Nurse Practitioner - Ambulatory Care & Prince County Hospital Units* The purpose of this position is to maintain, improve, restore and optimize the health status of clients within the ambulatory care services of PCH. This position may require some flexible shifts i.e. evenings and weekends on occasion. *Duties will include:* * perform comprehensive, focused health assessments including a health history and physical examination using a holistic framework and multiple assessment techniques relevant to a client?s stage of development, culture and health. * Determine the need, order, and interpret the results of relevant screening and diagnostic tests; Analyze assessment data by systematically comparing and contrasting clinical, behavioural and diagnostic data to determine differential and working diagnosis. * Determine, communicate and document health findings as well as medical and nursing diagnoses of specific diseases, disorders or health conditions. * Select and prescribe drugs and other therapies within scope of practice based on knowledge of pharmacokinetics, pharmacotherapeutics, drug interactions and individual patient characteristics. * perform invasive and non-invasive procedures as required within scope of practice. * collaborate with interdisciplinary teams and community members to identify patient population health needs and resources, and provide input as requested for community health programs. * discuss with and involve clients in decision making regarding their health, and in taking responsibility for their health. * provide direct guidance and counseling on health promotion and illness prevention strategies in accordance with client health status and needs, participate in the overall evaluation of the practice setting?s programs and services * ensure professional standards and clinical competence are reflected in all educational content taught by Primary Health Care Nurse Practitioner * willing to participate with in-patient care as appropriate; and perform other related duties as required. *Minimum Qualifications:* * must have a Masters? Degree in Nursing as a Nurse Practitioner or graduation from or completed the requirements to graduate from an approved and recognized All Ages Nurse Practitioner program. * extensive experience working as Registered Nurse is required. * active or eligible for registration with the College of Registered Nurses and Midwives of Prince Edward Island; * Successful completion of CPR within the last year is required * critical thinking proficiency to assess, analyze, diagnose, select treatment, evaluate, and refer patients with common acute or chronic health conditions, or for health promotion / illness prevention; * skilled in independent performance of advanced elective procedures; * demonstrated ability to work effectively in an interdisciplinary environment with excellent collaborative, team building and consultative skills; * excellent communication (verbal and written), facilitation and interpersonal skills in dealing with clients, families, community groups, professional colleagues, and referral partners; * thorough knowledge and proficiency in medical and nursing physical assessment and treatment methods; * demonstrated ability to prepare and present information on health promotion and illness prevention to the community and practice setting staff; * strong analytical, problem-solving and decision-making abilities; * the successful applicant must meet minimal productivity requirement; demonstrated ability to be self-directed; * must possess a valid driver?s license and have access to a reliable vehicle; * the successful applicant must have a good previous work and attendance record. *Other Qualifications:* * previous experience in leadership roles would be an asset; * additional relevant education and experience would be an asset * Must be available to work flexible hours and shifts such as evenings and weekends as required. *External candidates may be eligible to re
