Services are delivered through a multidisciplinary, person-centered, and family-focused model, ensuring coordinated care for both parents and children across systems. *Priority Populations* The Carter House will target families who: · Must relinquish care of their children to access treatment. · Are seeking treatment to regain custody of their children. · Are at risk of having their children apprehended due to substance use. *Job Summary ? Registered Nurse Manager* The Registered Nurse (RN) plays a critical leadership role within The Carter House, working within existing *wrap-around supports *through a *harm reduction and trauma-informed lens.* The multifaceted role combines clinical care, emotional support, and client education. This position is responsible for coordinating and overseeing medical care for both parents and infants, ensuring continuity, safety, and quality of care to includes medication adherence, managing MARs, fielding medical concerns, educating and coaching unregulated care providers, immunization and oral health clinics, facilitating medical appointments, conducting assessments, consulting with health care professionals, and coordinating referrals to Primary Health Care. The RN will collaborate closely with interdisciplinary staff, medical professionals, and community partners, and must demonstrate strong communication skills and cultural humility when working with clients from diverse social, economic, and cultural backgrounds. The RN must have the ability to communicate and maintain effective working relationships with clients from diverse social, economic, and cultural backgrounds, and maintain an effective working relationship with members of The Carter House team, as well as community health care providers and agencies. *Nursing responsibilities* *Clinical Care & Coordination* · Develop and maintain comprehensive medical care plans for all parents and children residing at The Carter House · Conduct comprehensive initial and ongoing physical, psychological, and psychosocial assessments · Provide medical support including screening, testing, treatment, monitoring, and follow-up · Coordinate care related to medication adherence, Medication Administration Records (MARs), medication reconciliation, and auditing. Medication management includes Medication-Assisted Treatment (MAT), such as methadone or buprenorphine. · Perform and follow up on toxicology screening, as required · Monitor medical concerns and consult with Pediatrics, Infectious Disease, and Primary Health Care providers · Support weekly physicians? clinic · Develop, implement, and adjust individualized care and treatment plans in collaboration with the clinical and interdisciplinary team (physicians, social workers, therapists, etc.). · Participate in development of Policies and Work Standards · Provide crisis intervention when clients experience acute distress, suicidal ideation, or other emergencies. · Provide education to clients on health and nutrition principles to support overall wellness. *Care Integration & Continuity* * Liaise with primary health care providers, hospitals, and specialists to support transitions and follow-up care * Coordinate with commu- Location Saskatoon, SK
