Job Description Social Worker II (Social Navigator), Permanent Full-TimeFamily Care Team, Coast of Bays, Location to be determinedThe philosophy of a Family Care Team (FCT) is to deliver care that is person centered, comprehensive, team-based, culturally safe, coordinated, accessible, and focused on quality and safety. This philosophy supports providers and care teams to meet clients where they are using an inter-professional collaborative approach where care is provided in the right place, at the right time, by the right provider in a manner that best suits the client's need. The team will ensure clients are treated with respect, dignity, and compassion, and enable strong and trusting relationships with providers and staff. FCT's will enable the delivery of a range of quality care for clients within their own communities while facilitating interdisciplinary work amongst healthcare providers.Definition of work: This is an advanced professional level position involving a high degree of responsibility, autonomy and independence. Within policy parameters, the Social Worker II accepts, screens, and actions referrals in relation to individuals who require supportive service. The Social Worker II will also provide case assessment and planning as well as emergency intervention, provide patient and family education; conduct risk assessments; assist clients in accessing required community services; help clients recognize, develop, and utilize support networks; and initiate appropriate community referrals. As part of an interprofessional team, the Social Worker II will complete assessments and interventions, while supporting and advocating for patients within their care, ensuring clients get the support they need to access healthcare and health-impacting resources. The Social Worker II will gather information related to economic barriers, healthcare system concerns, and basic needs (including but not limited to food, transportation, and material goods). Social Worker II will connect clients to vetted community agencies and resources which may include community-based support. The successful candidate must possess a strong knowledge of the social determinants of health, health promotion, and chronic disease management. The Social Worker II will also be responsible for developing, implementing, evaluating and continuously improving the role in this setting. The Social Worker II will facilitate and participate in team consultation, huddles and case conferencing. They will also serve on committees and be involved in program planning and development, as required. The Social Worker II reports to the Regional Manager for the Family Care Team. The Social Worker II will be a visible and enthusiastic champion for an improved client/patient/resident experience. Required qualifications:* Undergraduate degree in Social Work. * Current registration with NLASW.* Minimum of three years of experience within the last five working as a Social Worker in an acute care, long term care or community health care setting is required. * Experience in brief therapy modalities is an asset* Knowledge of quality improvement in health care (i.e. LEAN education and skills and/or demonstrated knowledge of the Model of Improvement). * Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, Medical Assistance in Dying, and the Children, Youth and Families Act.* Knowledge of mental health and addictions diagnoses and issues, housing and homelessness, healthy living, and health promotion strategies. * Knowledge of discharge planning and community-based resources. * Knowledge of crisis interventions and supportive counseling. * Knowledge of case management. * Considerable knowledge of the needs, resources and services available to seniors and individuals with disability.* Demonstrated ability in comprehensive assessment, interviewing and crisis intervention techniques, an
